Then he will x-ray the teeth to make sure they are healthy enough to withstand the small amount of enamel that is removed. The tooth may become weak if too much of the enamel is removed. The dentist will mark each tooth that is to be reshaped with a pencil. Sometimes the dentist may take a “before” picture to use as a reference. The pencil marks are used as a guide for sculpting with a sanding drill or laser. Rough strips are moved back and forth to shape the sides of the tooth, and then it is smoothed out and polished. It usually takes anywhere from 15 to 30 minutes for each tooth. An “after” photograph may be taken to compare and display the difference to the patient. Anesthetic is not usually required as the amount of enamel removed is not enough to cause pain or discomfort. There may be some sensitivity to hot or cold substances for a few days afterward but the slight irritation is temporary.

The result that the patient desires and dentist strives for is to change the shape or size of the teeth so that those which are not proportioned or slightly damaged can be brought into alignment with the others. Generally, this is considered a cosmetic procedure and usually involves the upper central, lateral and canine teeth. Sometimes it is performed after the removal of braces to fix any minor problems that were left unresolved by the braces. Contouring can also be used in conjunction with veneers and bonding.

One of the benefits besides a nicer looking smile is that the teeth become easier to clean after the overlaps and repairs have been made. This will ultimately reduce the risk of cavities and gum disease. It is a safe procedure with little to no risk to the patient. In some cases, the procedure may be covered by dental insurance in the case that the chipping or damage is the result of an accident. For those who are interested in contouring for purely cosmetic reasons, they can expect to pay anywhere from $100 to $500 per tooth.

How do I know if my child is in need of orthodontic treatment?

It is usually difficult for you to determine if treatment is necessary because there are many problems that can occur even though the front teeth look straight. Also, there are some problems that look intimidating and complex which will resolve on their own. Asking your general dentist is good reference, but we are your best resource since orthodontics is all we do. What are the early symptoms of orthodontic problems?

Although determining if treatment is necessary is difficult for you to assess, the following symptoms may help in prompting you to seek our orthodontic advice.

Ask your child to open their mouth, and let you look at their teeth. If you see any signs of crooked teeth, gaps between your child’s teeth or overlapping teeth, your child may need orthodontic treatment.

Ask your child to bite all the way down, but keeping their lips open so you can see their teeth. Do the front top teeth line up with the bottom? Do the top teeth protrude out away from the bottom teeth? Do the top front teeth cover more than 50% of the bottom teeth? Are the top teeth behind the bottom teeth? All these are indicators for potential orthodontic treatment.

Look at the alignment of your child’s jaw. Does the jaw shift off center when your child bites down? If you see any misalignment or shifting of the jaw, your child may have a skeletal problem, which requires early orthodontic intervention.

These are only some of the obvious symptoms of orthodontic problems.

What age should my child be seen by an orthodontist?

The American Association of Orthodontists recommends that your child be evaluated by age seven. Early detection of some orthodontic problems is important in order to take early corrective action and avoid more difficult treatment later.

Can you be too old for braces?
No, age is not a factor, only the health of your gums and bone which support your teeth. About 25% of our orthodontic patients are adults and that number is still growing!

Will it hurt?
Orthodontic treatment has improved dramatically. As a rule, braces make your teeth sore for a few days, but it is not painful. This annoyance can be relieved with an over-the-counter pain reliever. Today’s braces are smaller, more comfortable and use technology that reduces the discomfort. We use the latest in miniature braces and the highest quality of orthodontic materials in order to reduce discomfort and treatment time.

Can I still have braces if I have missing teeth ?
Yes. When teeth are missing, adjacent teeth will drift into the empty space. This will cause a functional, esthetic or periodontal problem. Orthodontic treatment will correct and prevent these problems from happening and will also provide proper alignment for your dentist to replace the missing teeth.

Questions About Treatment

What is Phase I (Interceptive Treatment) and Phase II treatment?
Phase I or Interceptive Treatment usually starts while the child has most of their baby teeth and a few of their permanent front incisors. This stage in development is usually about the age of seven. The goal of Phase I treatment is to intercept a moderate or severe orthodontic problem early in order to reduce or eliminate it. These problems include skeletal dysplasias, crossbites and crowding. Phase I treatment takes advantage of the early growth spurt and turns a difficult orthodontic problem into a more manageable one. This helps reduce the need for extractions or surgery and delivers better long term results and treatment options. Most Phase I patients require a Phase II treatment in order to achieve an ideal bite.

Phase II treatment usually occurs a number of years later. Usually we are waiting for 12-16 more permanent teeth to erupt before Phase II begins. This most commonly occurs at the age of 12 or 13. The goal of Phase II treatment is to achieve an ideal occlusion with all of the permanent teeth.

What is Full or Comprehensive Orthodontic Treatment?

This is another name for orthodontic treatment in the permanent dentition at any age. It is more commonly used when a Phase I treatment was not performed.

Does everyone need a Phase I treatment?
Absolutely not! Only certain bites require early intervention. All others can wait until most if not all their permanent teeth erupt.

Can I wait on Phase I/Interceptive Orthodontic Treatment until my child is older?
This is not recommended. If your child needs Phase I treatment this usually means that he has a difficult problem that requires attention now. If no orthodontic action is taken, treatment options become limited, more difficult, and the long term stability may be compromised. In addition, it may lead to extractions, oral surgery and increase costs.

What is the length or duration of orthodontic treatment?

Braces may be on between 6 months to 30 months, or longer depending on the age of the patient, the severity of the problem, the patient’s cooperation, and the degree of movement possible.orthodontics san antonio

What are extraction and non-extraction therapy, and what are the advantages and disadvantages of each?
Extraction therapy is a technique where some teeth are removed to make room for the other teeth in your child’s mouth. This is in contrast to non-extraction therapy where one expands a patients’ jaw and shave down some teeth to make everything fit.

What are lingual braces?

Lingual braces are mounted behind a patient’s teeth. They were used many years ago, before the advent of orthocosmetic ™ braces. Lingual braces are rarely used anymore. Generally, lingual braces are more uncomfortable than standard braces. Orthodontic treatment takes twice as long and is more costly. In addition, some people have trouble talking with lingual braces. Presently, Lingual braces are only used in special clinical cases.

Will I pay extra for designer braces, colored ligatures and brackets or flavored rubber bands?

Questions About TMJ Disorder and Wisdom Teeth

Can orthodontic treatment help me with Temporomandibular Joint (TMJ) problems?
There are some associations with a bite that is not correct (malocclusion) and TMJ disorders. TMJ disorders are a family of problems related to your complex jaw joint. Do wisdom teeth affect crowding of teeth after braces have been removed?
Unfortunately, the answer is not entirely clear. There are data to support and refute the claims that wisdom teeth cause crowding after braces have been removed. For this reason, we recommend wearing retainers full time after removal of braces. The orthodontist will tell you when you can switch to nighttime only wearing. Each patient is different so the time interval to go to nighttime only depends on the individual case. Always follow exactly what the orthodontists have told you to do.

The average adult has thirty-two teeth by age eighteen: sixteen teeth on the top and sixteen teeth on the bottom. Each tooth in the mouth has a specific name and function. The teeth in the front of the mouth (incisors, canine and bicuspid teeth) are ideal for grasping and biting food into smaller pieces while the back teeth, or molar teeth, are used to grind food up into a consistency suitable for swallowing.

However, the average mouth is made to hold only 28 teeth. It can be painful when 32 teeth try to fit in a mouth that holds only 28 teeth. These four other teeth are your Third Molars, also known as “wisdom teeth.”

Why Should I Remove My Wisdom Teeth?

Wisdom teeth are the last teeth to erupt within the mouth. When they align properly, and gum tissue is healthy, wisdom teeth do not have to be removed. Unfortunately, this does not generally happen. The extraction of wisdom teeth is necessary when they are prevented from properly erupting within the mouth. They may grow sideways, partially emerge from the gum, and even remain trapped beneath the gum and bone. Impacted teeth can take many positions in the bone as they attempt to find a pathway that will allow them to erupt successfully.

These poorly positioned impacted teeth can cause many problems. When they are partially erupted, the opening around the tooth allows bacteria to grow and will eventually cause an infection. The result: swelling, stiffness, pain and illness. The pressure from the erupting wisdom tooth may move other teeth and disrupt the orthodontic or natural alignment of teeth. The most serious problem occurs when tumors or cysts form around the impacted wisdom tooth, resulting in the destruction of the jawbone and healthy teeth. Removal of the offending impacted tooth or teeth usually resolves these problems.

Why Invisible Braces Are Beneficial For Both Oral Hygiene And Self-Confidence

In my last article I explained the immediate and most apparent advantages of invisible braces or clear braces compared with traditional metal or wire braces. Clearly for most people the obvious word is ‘invisible’, and being able to wear braces which are virtually impossible to see unless you actually look directly at someone’s teeth has to be of huge benefit to most people.

Wearing traditional, metal braces can certainly be damaging to a person’s confidence. The fact that statistically the majority of people who wear braces are in their teens, a time when people’s confidence is at its weakest, wearing braces which are invisible is certainly of great benefit. For many people the reason why they choose invisible braces is simply because of this fact alone, but there are many other advantages with wearing invisible braces other than the fact that they are hard to spot.

In this article going to look at another of the advantages which clear braces offer those people who feel their teeth could do with some alignment. Many traditional metal braces are fixed into place, so that they can provide consistent level of pressure against the teeth over the period of time they need to be worn. This offer represents a problem to people, because clearly having metal braces fixed your teeth for two or more years makes it difficult as far as oral hygiene is concerned.

Cleaning teeth is not easy with a metal brace in place, and there are even some foods which can be tricky to eat. If you’re someone who enjoys eating seeded buns then you may find that whilst you’re wearing a metal brace you choose not to eat that particular item, simply because it is too easy for the seeds to become lodged or trapped within the metal brace. Not only does this feel awkward and uncomfortable, but of course it does represent an issue as far as dental hygiene is concerned.

But of course there’s also the fact that there may possibly over the next couple of years be occasions when you really don’t want to have a metal brace defining who you are and what you look like. Perhaps you’ve been wearing a metal brace throughout your time at university, and you really don’t want all your graduation photographs to be dominated by a metal framework grinning at anyone who ever sees your picture.

Whether it’s still photographs, family photographs, passport photographs or any other kind of photograph that is important to you, it is certainly beneficial if you can make do without having a metal brace which is highly visible. The advantage then with invisible braces or clear braces is that they can be removed at any time, very easily.

Invisible braces are actually clear trays which simply fit over your upper or lower teeth. These clear trays are not only quite comfortable to wear, and almost completely invisible, but can easily be slipped on or off at any time. I should point out that taking invisible braces off is not recommended, and you must always remember to put them back on again as soon as possible. Wearing invisible braces for anything less than 23 hours a day could well result in your teeth actually shifting back into their original position, making it almost impossible to fix the invisible braces back. This could mean having to start the process all over again.

So being able to remove invisible braces for particular times such as a special meal, for cleaning your teeth, or the special photographs, is an advantage which has often been one of the deciding factors for many people who have chosen to have invisible braces or clear braces fitted rather than traditional metal or wire braces. General Soreness

When the orthodontic appliances are placed, there may be a feeling of general soreness in the mouth, and teeth may be tender to biting pressures for a few days. This can be improved by rinsing with a warm saltwater mouthwash. Dissolve one teaspoon of salt in 8 oz. of warm water and rinse vigorously. If the tenderness is severe, take whatever is normally taken for headaches or similar pains. The lips, cheeks, and tongue may also become irritated as they become accustomed to the surface of the braces. Wax can be placed on the brackets to lessen irritation. After the appliances are placed, we can provide instructions in proper appliance care and tooth brushing.

Loosening of Teeth

This is to be expected throughout treatment. Teeth must loosen so they can be moved.

Loose Wire or Band

Don’t be alarmed if a wire or bracket comes loose. This happens occasionally. If a wire protrudes and is irritating, use a blunt instrument (back of spoon) and carefully push the irritating wire under the arch wire. If irritation to the lips or mouth continues, place wax or wet cotton on the wire to reduce the annoyance. Callus as soon as possible for an appointment to check and repair the appliances. If any piece comes off, save it and bring it with you to the office.

Braces & Sports

If sports are played, it’s important to consult Dr. Rose or Dr. Malik to learn about the special precautions needed while playing sports during orthodontic treatment. A protective mouth guard specifically designed for use with orthodontic appliances must be used while playing contact sports. Discuss sports activity with Dr. Cochran and they will select the appropriate mouth guard. In case of any accident involving the face, check the mouth and the appliances immediately. Emergencies

True orthodontic emergencies are very rare, but we can help when they do occur. If there is severe pain or if there is a painful appliance problem that can’t be taken care of by the patient or parent, call us to schedule an appointment. When describing the appliance, it is helpful to know the names of the orthodontic appliance parts to explain what may be broken or out of place. It is very important you call as soon as possible to schedule a time to assess the problem.

Allowing the appliance to remain damaged for an extended period of time may result in disruptions to the treatment plan. Ask us to demonstrate how to handle minor orthodontic appliance problems. Many times, a bit of wax can be placed in an area of discomfort to provide temporary relief. The following solutions may help relieve discomfort:

Poking Wire

Using a pencil eraser, push the poking wire down or place wax on it to reduce the discomfort. Wax can be applied on the braces to help reduce mouth irritation. Ask Donna, Karen, Maria, or Randa to demonstrate how to apply the wax and how to handle minor orthodontic appliance problems. You may also call the office to schedule an appointment to resolve the poking wire issue.

Loose Bracket or Band

If the bracket or band is still attached to the wire, leave it in place and put wax on it. If the wire comes out entirely, wrap the bracket with a tissue and notify the office right away.

Loose Wire

Using tweezers or a small pair of needle-nose pliers, try to place the wire back into place. If doing this and using wax does not help, as a last resort use a small fingernail clipper to clip the wire behind the last tooth to which the wire is securely fastened. If discomfort continues, place wax on it, and call the office to schedule a repair appointment.

Lingual braces work in much the same way as their traditional counterparts. However there’s one important cosmetic difference; the lack of the usual metal supports on the front of your teeth. In the past braces could only be fit in a certain position; right on the front of your teeth but due to developments in dental technology this is no longer the case. The key difference with Lingual Braces is that they can be placed on the rear of your teeth, hiding the majority of the metal parts. How are Lingual Braces fit?

Firstly, your dentist will need you to press your teeth into a mould in order to take an impression of them. This mould is then sent off to a specialist laboratory where each bracket in your lingual brace is custom made to fit your teeth. In order to help the brackets remain in the correct shape they are usually set in wax and kept safely inside a plastic applicator. During your follow-up visit your dentist will transfer the brackets to the back of your teeth and cement them in place. There are many factors to consider when deciding whether to get invisalign or traditional metal braces. Several of the key comparisons are the following:

Visibility

The great advantage of invisalign over metal braces is that it is barely visible. This is obviously not the case with metal braces, as they are visible the moment you open your mouth.

Cost

Invisalign tends to cost more than metal braces, as the technology behind it is more expensive. If pricing plays an important role when deciding between the two, do some thorough research. Pain

Invisalign doesnt involve metal that can irritation your teeth and gums, while metal wires and brackets used in braces can cause irritation. Pain is generally minimal with invisalign. Most patients feel pain only in the first day or two of each new tray change. Because invisalign trays are changed more frequently than metal braces are tightened (2 weeks vs. 6 weeks), the adjustment and movement with invisalign is more gradual, and so there is much less pain to bear. What I prefer to do each time I need to change trays is to change in the evening before bedtime. I take 2 tylenols, put on the new tray, and by the morning time, the tray has had time to loosen up and becomes a bit easier to remove.

Hygiene

Teeth hygiene is very important, and in this case, invisalign is the clear winner. Because invisalign is removable, you can brush and floss your teeth as thoroughly as you normally would. Unfortunately, this is not the case with braces. You cannot floss for the whole duration of your braces treatment. This results in food particles getting stuck in between your teeth and very possibly resulting in decay. This is not to say that there are no risks with invisalign. In fact, in my research through dental blogs and forums, I have come across some people who reported getting several cavities during their invisalign treatment. This could be just coincidental, or it can be a result of the invisalign. If you practice good hygiene and follow the basic instructions of invisalign, you should be fine. You are not supposed to eat or drink anything while wearing your trays with the exception of water. If you drink a soda or anything other than water, that sugar will get trapped in the trays and as you may know, wetness is a breeding ground for bacteria. It is important to adhere to the water only rule AND to remember to brush after every meal so that food and sugars do not linger and cause possible cavities or decay.

Eating

Since you remove your invisalign trays for all meals, you can go on eating and drinking anything you’d like. Traditional braces limit you in what you can eat since certain foods are more likely to get trapped and stuck in the wire.

Speech

When first starting invisalign, you might have a slight lisp for a few days until your tongue gets adjusted to the new material in your mouth. This usually passes pretty quickly for most people and is hardly noticeable to others.

Initial Examination

When you visit our office for an initial orthodontic evaluation, the orthodontist will perform a comprehensive examination to determine if orthodontic treatment is needed and present the initial findings to you that same day. During the exam the doctor will evaluate jaw, teeth and facial growth to determine if now is the best time to begin orthodontic treatment. If additional growth is recommended, prior to beginning orthodontics, the orthodontist will place your child in the practice recall system and continue to monitor your child as necessary as the permanent teeth erupt and the jaws and face continue to grow. If your child or yourself is ready to begin orthodontics immediately the doctor along with the practice treatment coordinator will explain to you the recommended treatment plan, show you the types of appliances available to treat your particular case, review estimated length of treatment, provide a cost estimate and work with you to customize a payment plan that fits your needs, and scheduled to have your appliances placed.Children orthodontics san antonio

Early Treatment

When necessary early treatment or interceptive treatment may be recommended to guide facial growth and tooth eruption to prevent more serious problems from developing. Early intervention frequently makes the completion of treatment at a later age easier and less time consuming. A variety of early treatment appliances are now available to help your child correct an orthodontic problem. One example is an appliance that expands the palate while the jaw and face are still growing to make room for more teeth and create a broader smile. Diagnostic Records

Following the initial examination, diagnostic “Records Will be taken on each patient. Records confirm the doctor’s diagnosis and treatment plan and serve as documentation of the position of your teeth, lips and profile when you started treatment. Typically records include the following five items:

Panoramic x-ray – This type of x-ray takes captures an image of your teeth and jawregion. It helps in identifying any extra teeth, blocked out teeth, missing teeth, or any problems with your jaws, jaw joints or roots. Depending on your particular problem you may have to have several Panoramic x-rays taken throughout treatment.

Cephalomatic x-rays – This type of x-ray captures an image of your entire skull and profile. It is used to identify any potential growth problems and for recording diagnostic measurements.

Bite registration – Typically your bite registration is taken by having you bite your teeth together into a piece of thin wax. This allow your orthodontist to evaluate how well your top teeth line up with the bottom teeth.

Impressions/ Study Models – In order to make plaster models of your teeth (study models), you will need to have impressions taken as part of the records process. This entails biting into a tray of soft toothpaste or pudding like material (alginate) for about one minute. The material becomes set (or hardened) to a consistency of silly putty and then is taken out. Impressions are taken one at a time and you need one for the top and one for the bottom. This process is painless but can sometimes taste bad. In order to make this more enjoyable we offer a variety of alginate flavors for you to choose from.

Diagnostic photographs – These are pictures taken of your smile, profile, face, teeth and mouth. These pictures are again used to document your current status and will help the doctor track your progress and see how your smile is changing.

Final Treatment Plan
After the records have been reviewed, we will discuss with you and your child the final orthodontic treatment plan including what orthodontic appliances will be used, how long treatment will last, what foods to avoid, proper care of the appliances and teeth during treatment, etc.

Length of Treatment
The length of treatment for each patient varies based on the patient’s age, severity of the bite, and correction needs, but most cases can be completed in approximately 18-24 months. After the braces are removed, a retainer is prescribed and is to be worn continuously for a few months to a few years in order to set the tissues surrounding the newly straightened teeth. When ready, your orthodontist will inform you that you can now just wear the retainer at night or occasionally to make sure the teeth do not shift. In some cases, a permanent retainer can also be prescribed, which is a metal wire that is bonded to the back of the teeth to insure that the teeth remain in proper position.

Patient Cooperation
Patient cooperation is the key to successful orthodontics. Braces are strong enough to withstand the normal forces created during chewing, however, if they are abused, the braces can break. Broken braces and wire may poke into the cheek or tongue and cause sores in the mouth. Broken appliances can also lengthen the treatment time. Appliances must also be worn as prescribed to achieve the desired results.

Dental hygiene is also very important to prevent tooth decay or decalcification during treatment. Certain foods should be avoided to prevent breakage of appliances and also reduce the chance of tooth decay. We will discuss this with you in more detail on the day the appliances are placed, but for more information, please go to the “Caring for Braces” section.

1. Why do people need braces?
There are many reasons one person may need braces: crowded teeth, missing teeth, extra teeth, jaws not growing right, bad overbite, open bite and the list continues. 2. What is the best age for braces?
There is NO best age for braces as all ages can be treated. However, the best time to be seen by an orthodontist is around 7 years old. At 7, any jaw growth problems can easily be corrected.

3. What is Phase I Treatment?
It means that if a problem exists (such as a bad overbite or a lot of crowding of teeth) treatment can or will be started as early as 7 or 8 to correct the problem. Most of the time, Phase I Treatment can save the patient from extraction of good permanent teeth, headgear or major jaw surgery.

4. What is Phase II Treatment?
Phase II Treatment is treatment with “braces”. Patients who have had Phase I Treatment to take care of a major problem (severe overbite or crowding) may need some type of Phase II Treatment with braces to detail their bite. However, not everyone needs Phase II Treatment. For someone who does not have Phase I Treatment but needs braces, they go directly to Phase II Treatment (that is, treatment with full braces).

5. How do braces work?
Braces work by putting light forces on teeth through special wires. Orthodontists also use special rubber bands to bring teeth into the right position. Treatment takes some time because teeth move slowly (only 1mm a month or less) into the right position.

6. Does it hurt to put braces on?
No, the braces are put on with special cement. However, in the beginning, teeth may become a little sore once they start moving.

7. How long does it take to put braces on?
The appointment to put the braces on is 1½ hours but to actually cement the braces in place will take approximately 10-15 minutes.

8. How long does treatment take?
Depending on the severity of a case, it could take as little as 6 weeks or up to 20-24 months for complete treatment. Once treatment is started, a patient would be seen every 8-12 weeks depending on their needs.

9. Do you talk funny with braces?
Just like anything new, it takes about a week to get used to the feeling. Most people adjust very well and have no problems.

10. Can you play sports with braces?
Yes, but with much care. You may need a mouth-guard to prevent injury and damage to the braces, your teeth and gums. We will gladly provide one for you.

11.Can I eat anything with braces?
No. There are some restrictions. We ask that you avoid sticky or hard foods (such as caramel, sticky gum, candy, popcorn, etc.). A little extra care with what you eat can go a long way and can prevent braces from breaking or becoming loose.braces San Antonio

12.Can you get a “shock” if braces are touched with something metal (fork, spoon, etc.)?
Usually not.

13.Can the braces set off alarms in stores or airports?
No, the metal is stainless steel and does not set off alarms.

14. What are rubber bands and why do people need them?
Rubber bands bring the teeth together to make them fit perfectly. Most people need them at the end of treatment for a couple of months.

15. What are retainers and who needs them?
After treatment is finished, retainers are given to hold the teeth in position until they become used to their new “home”. All patients need retainers and Dr. Petrol will instruct you to wear them according to your personalized needs.
